e. An acceptable written agreement with the owner(s) as to affordability and <br /> subsequent rate increase,if rental property is involved. <br /> The information must be maintained for review and verification during on-site <br /> monitoring visits; and, <br /> 8. The following data will be provided by housing unit as part of the administrative <br /> closeout for each NSP activity providing direct benefit (i.e. down payment <br /> assistance, temporary relocation, rehabilitation, etc.) and summarized by activity <br /> and submitted with the administrative closeout package: <br /> a. Address of each housing unit acquired and/or rehabilitated with NSP <br /> funds, the date the construction or sale was completed on the housing unit, <br /> and the amount of NSP funds spent on that housing unit; <br /> b. Whether the household is headed by a female, the number of handicapped <br /> persons in the household, the number of elderly persons in the household, <br /> and the LMMI status of the household; <br /> C. The number of occupants in the household, categorized by gender; and <br /> d The racial demographics of the household by number (white, black, <br /> Hispanic, Asian/Pacific Islander. Hasidic Jew or American Indian/Alaskan <br /> native. <br /> 35 <br />
