V40- Program Description: A family in need of services completes a Whole Child Profile. The <br /> Whole Child web system matches family needs to providers with appropriate services. Families <br /> will develop and implement a holistic plan to meet their needs and select and contact providers. <br /> Providers deliver services to the family and the Whole Child system tracks the outcomes. <br /> This program is designed to: connect families to parent education classes, childcare <br /> programs, community-based programs that address adolescent's needs, after-school programs <br /> and programs for high risk expectant and new mothers. With the data collected from the Whole <br /> Child Connection, agencies and funders can determine gaps in services and better address the <br /> needs of families. <br /> Collaboration: Connected 4 Kids, Indian River School District, 211 Palm Beach/Treasure <br /> Coast <br /> Outcome # 1: In FY11/12 increase to 75% the number of participants who answer the Client <br /> Satisfaction Survey who strongly agree or agree that the Whole Child Profile addressed most of <br /> their concerns. Baseline: Martin County had 98% answer they were satisfied. <br /> Result: Goal not met. <br /> Of those families who used the Whole Child Connection and completed a satisfaction survey <br /> 60% indicated they agreed or strongly agreed that the Whole Child Profile addressed most of <br /> their concerns. Thirty percent neither agreed nor disagreed and only 10% disagreed. <br /> Outcome #2: In FY11/12 increase to 75% the number of participants who answer the Client <br /> Satisfaction Survey who report greater ease when accessing services as a result of completing <br /> the Whole Child Profile. Baseline: Martin County had 97% answer they were satisfied. <br /> Result: Goal met. <br /> Of those families who used the Whole Child Connection and completed a satisfaction survey <br /> 100% indicated they strongly agreed or agreed that it is easier to access services. <br /> Outcome#3: In FY11/12 increase to 75% the number of service requests that are answered by <br /> a Whole Child Connection provider. Baseline: Martin County had 98% of their service requests <br /> answered. <br /> Result: Goal not met. <br /> Whole Child is actively working to obtain additional provider agreements. Currently 25 providers <br /> have signed service agreements. Data results will be available as the number of provider <br /> agreements increases. <br /> Service Capacity: <br /> Through the fourth quarter(100%) of the 2011-2012 contract, Whole Child has seen: <br /> Goal Actual % of Goal <br /> Families 1000 298 29.8% <br /> - 29- <br /> 64 <br />
