Kafer,Jr., P.E., County Enginee <br /> �w <br /> SUBJECT: FDOT Local Agency Program (LAP) Supplemental Agreement <br /> and Resolution Authorizing the Chairman's Signature <br /> Old Dixie Highway Sidewalk from 8th Street to 20th Street <br /> I IRC Project No. 1102 - FM#: 425710-1-58-01 <br /> DATE: September 13, 2013 <br /> DESCRIPTION AND CONDITIONS <br /> On May 22, 2012, the Board of County Commissioners entered into a Local Agency Program <br /> Agreement(LAP)FM#425710-1-58-01 with the Florida Department of Transportation(FDOT)for <br /> construction of a 5-foot wide concrete sidewalk along the east side of Old Dixie Highway from 8th <br /> Street to 20th Street in the amount of$934,475.00. Timothy Rose Contracting, Inc.was awarded <br /> the bid for this project, with Change Order No. 1 on November 13, 2012 in the amount of <br /> $329,327.97. A Supplemental Agreement was approved by the Board on November 20, 2012, <br /> reducing the amount of the grant with the FDOT from$934,475.00 to$347,930.87(which includes <br /> construction engineering and inspection services). <br /> Change Order No. 2 was approved by the Board of County Commissioners on August 20, 2013 <br /> increasing the contract amount by $37,473.97 for various field changes that were made <br /> throughout the project including drainage, driveway and minor landscape modifications, <br /> additional hand rail, curb replacements, curb ramp deletions/additions and miscellaneous <br /> sidewalk re-alignments to avoid unforeseen conflicts, bringing the contract total to$366,801.94. <br /> The FDOT has requested Indian River County execute and deliver to the FDOT the attached <br /> Supplemental Agreement for the addition of LAP funds in the amount of$37,474.00 for Change <br /> Order No. 2. Also attached, please find a Resolution authorizing the Board of County <br /> Commissioners to execute this Local Agency Program Supplemental Agreement for LAP Contract <br /> No. AQN-64 (FM #425710-1-58-01). <br /> FUNDING <br /> Funding for this project is budgeted and available from the Optional Sales Tax — 31521441- <br /> 066510-12814, Old Dixie Highway Sidewalk(8th Street to 20th Street) and is being funded by the <br /> FDOT Local Agency Agreement FM 425710-1-58-01.
