ATTACHMENT "A" SUPPLEMENTAL JOINT PARTICIPATION AGREEMENT Deliverables: 1.) Refer to Exhibit C and Exhibit D of original JPA. �.► 2.) Comply with Transparency Act where applicable. 3.) Recipients providing fixed -route service can segregate urbanized and non -urbanized miles based on route maps and allocate system -wide costs accordingly, driver's logs can be maintained to segregate mileage or hours inside and outside the urbanized area. 4.) Certification verifying adoption of a System Safety Program Plan. 5.) Quarterly reports submitted via TransCIP. 6.) Submit invoice every three (3) months at minimum. 7.) A copy of the most recent active CTC agreement, if applicable and requested by FDOT project manager. Public Records: The Agency shall allow public access to all documents, papers, letters, or other material subject to the provisions of Chapter 119, Florida Statutes, and made or received by the Agency in conjunction with this Agreement. Specifically, if the Agency is acting on behalf of a public agency the Agency shall: (1) Keep and maintain public records that ordinarily and necessarily would be required by the Department in order to perform the services being performed by the Agency. (2) Provide the public with access to public records on the same terms and conditions that the Department would provide the records and at a cost that does not exceed the cost provided in Chapter 119, Florida Statutes, or as otherwise provided by law. (3) Ensure that public records that are exempt or confidential and exempt from public records disclosure requirements are not disclosed except as authorized by law. (4) Meet all requirements for retaining public records and transfer, at no cost, to the Department all public records in possession of the Agency upon termination of the contract and destroy any duplicate public records that are exempt or confidential and exempt from public records disclosure requirements. All records stored electronically must be provided to the Department in a format that is compatible with the information technology systems of the Department. Failure by the Agency to grant such public access shall be grounds for immediate unilateral cancellation of this Agreement by the Department. The Agency shall promptly provide the Department with a copy of any request to inspect or copy public records in possession of the Agency and shall promptly provide the Department a copy of the Agency's response to each such request.
