The Customer must <br /> provide a contact name and phone number for use by locate services should they need <br /> to gain access to a secured construction area or are unable to find the site based on <br /> locate info provided by Customer. <br /> 3. Pricing includes a default-generated graphical screen at the central site. Any custom <br /> graphical screen requirements will require a change order. <br /> 4. This quotation assumes that entry by our employees into "Confined Spaces" and/or <br /> "Permit-Required Confined Spaces" as defined by OSHA is not required by the scope <br /> of the work to be performed under this quotation. <br /> 5.
