DIRECTOR <br />DEPARTMENT OF GENERAL SERV ES <br />'SUBJECT: INDIAN RIVER COUNTY PUBLIC HEALTH UNIT <br />FINAL CHANGE ORDER - NUMBER 6 <br />BACKGROUND: <br />poop 8 F',�, 77 <br />The following items are hereby 'submitted as Change Order Number <br />on the subject project, which will necessitate a change in <br />contract with Miorelli Engineering and Indian River County: <br />six <br />the <br />Item No. 1 - <br />Additional Cabinets. Architect determined extra <br />cabinets were required but the elevations were missing <br />in a few rooms and detail was not adequate. Cost of <br />$1,801.79 and no additional time. <br />Item No. 2.- • <br />Credit for Maple Trees. Fourteen trees did not meet'''.,*' <br />".• .` <br />' s ecified <br />p grade. A credit of $2,124.00 was given the <br />County. <br />Item No. 3 -. <br />Add 4 fire alarm chime/strobes. During fln'al <br />inspection the Fire Marshall indicated the fire alarms <br />did not most the decibel .level required. Cost of. <br />. <br />$1,800.00 and no additional time. <br />Item No. 4 -;` <br />Revise retention pond. After completion the retention' <br />pond did not function as designed. -Additional work <br />was required to remove poor soil and replace with sand <br />that would accommodate percolation. Cost -of <br />$15,938.00. <br />Item No. 5 - <br />Credit for monies not used in hardware and graphic t <br />allowances. Credit of $6,875.12. <br />Item No. 6 - Change Order No. 5, authorized removal and replacement <br />of Doors 118A and 118B. The cost' was less than <br />anticipated. Credit of $1,451.00. <br />Net affect on the original contract is an increase of $9,089.67 or a <br />total contract price of $2,453,995.67. <br />RECOMMENDATIONS: <br />Staff recommends approval of Change Order Number Six and requests <br />authorization for the'Chairman to execute the applicable documents. <br />.4 <br />ON MOTION by Commissioner Eggert, SECONDED by <br />Commissioner Wheeler, the Board unanimously approved <br />Change Order No. 6 for IRC Public Health Unit and <br />authorized the Chairman to sign the document, as <br />recommended by staff. <br />SAID CHANGE ORDER IS ON FILE IN THE OFFICE OF <br />CLERK TO THE BOARD <br />18 <br />
