WORK ORDER NUMBER 7 <br /> i 66th Avenue Roadway Widening - 491h Street to 8111 Street <br /> IRC Project No. 1505 <br /> This Work Order Number 7 is entered into as of this 5 day of juLti 2016, pursuant to <br /> that certain Continuing Contract Agreement for Professional Services, dated November 15, 2011 and that <br /> certain Extension and Amendment of Continuing Contract Agreement for Professional Services entered into <br /> as of the 4th day of November, 2014(collectively referred to as the "Agreement"), by and between INDIAN <br /> RIVER COUNTY, a political subdivision of the State of Florida ("COUNTY") and Kimley-Horn & <br /> Associates, Inc.("Consultant"). <br /> EXHIBIT A <br /> SCOPE OF WORK <br /> The COUNTY desires to complete the design,construction document development and permitting associated <br /> with the widening of 66th Avenue to a four lane divided corridor as previously approved by the COUNTY <br /> through the adoption by the COUNTY Commission of the 66th Avenue/ Schumann Drive Corridor Study, <br /> presented :in 2010. Consultant services are required for the preparation of design surveys, roadway <br /> construction documents and review agency permits necessary to support widening 66th Avenue between <br /> 491h Street to 81St Street, with associated side street improvements. The COUNTY desires to develop the <br /> improvements within two independent construction phases,the remaining portion of Phase I being between <br /> 491h Street to 57th Street and Phase II being between 57th Street to 81St Street. <br /> i <br /> A. Proiect Management/Coordination: <br /> 66th Avenue Phase I Improvements has been divided between two separate consulting firms,the COUNTY <br /> has requested that the two firms coordinate to represent a single set of coordinated construction <br /> documents. Based upon a meeting with COUNTY staff, the other consulting firm and Kimley-Horn; <br /> Kimley-Horn has agreed to serve as the project coordinator responsible for delivering the coordinated <br /> construction plans to the COUNTY. <br /> I <br /> B. Roadway Analysis and Plans Revisions: <br /> The Consultant will revise the Phase I and Phase II construction documents, quantities, etc. to reflect <br /> the corridor transition between Phase I and Phase II improvements, in addition the Phase 11 construction <br /> documents will be revised to reflect the transition from a four lane divided corridor to the existing two <br /> lane corridor north of 811t Street. <br /> C. Utility Coordination: <br /> The Consultant will coordinate with franchise utility operators in the vicinity of the project such that the <br /> proposed construction activities can be developed to minimize impacts to existing utilities located within <br /> the project limits. Any utility permitting and relocation design required to accommodate the proposed <br /> improvements will be addressed as an additional service relative to this scope of work. <br /> D. Legal Description and Sketch Revisions: <br /> The Consultant will update/ revise the previously submitted 66th Avenue and associated side street legal <br /> description and sketches, based upon requests by the COUNTY as a result of coordination with the <br /> respective property owners. <br />
