E., Roadway Production Manager �v <br />SUBJECT: Amendments 1, 2, and 3 to Work Order No. 1— Engineering - Carter Associates, Inc. <br />415` Street/US1 Intersection Improvements <br />DATE: April 21, 2016 <br />DESCRIPTION AND CONDITIONS <br />On November 15, 2011, the Board of County Commissioners approved the Continuing Contract for <br />Professional Services with Carter Associates, Inc. and on November 4, 2014, approved an Extension and <br />Amendment of the Continuing Contract. <br />Work Order No. 1 provides design and permitting for construction of an eastbound left turn lane at 4151 <br />Street and US1 intersection, which will require modification of the existing US1 signalization components to <br />accommodate the additional vehicular lane, for a lump sum amount of $86,645.00. This additional left turn <br />lane will extend approximately 1,100 feet to west of the Florida East Coast Railroad right-of-way. <br />Amendment No. 1 provides additional professional engineering services for the Indian River County Utility <br />Department for design of water and sewer utility relocations and adjustments addressing utility conflicts <br />with the proposed roadway improvements. The scope of work is shown in Exhibit A of this amendment in the <br />amount of $11,260.00. <br />Amendment No. 2 will provide professional engineering services to redesign the intersection of 4111 Street <br />and US1, avoiding extensive and costly work within the right-of-way of US1. This amendment includes the <br />design of two signal mast arm assembly upgrades necessary for the new alignment.
