Board of County Commissioners <br /> 180127th Street <br /> Vero Beach, Florida 32960 <br /> Telephone: (772) 567-8000 FAX: (772)-778-9391 - <br /> Professional Design Services <br /> For <br /> Indian River County Jail — Reroofing Project <br /> WORK ORDER NO. 2 (Facilities Management Division) <br /> FOR <br /> AMENDMENT TO CONTINUING CONTRACT AGREEMENT <br /> FOR PROFESSIONAL SERVICES, APRIL 18, 2014 - <br /> WITH <br /> JAY AMMON ARCHITECT, INC. <br /> All services performed under this contract shall be supervised and certified by a licensed Professional <br /> Engineer(PE) licensed with the State of Florida and staff employed directly with your firm. <br /> Scope of Services <br /> Indian River County is requesting that Jay Ammon Architect, Inc., prepare construction drawings <br /> consisting of roof plans and details; which depict the Scope of Work, assist in the bidding phase, and <br /> provide construction administration services for repair and replacement of various roofs at the <br /> Indian River County Jail, 4055 41St Avenue,Vero Beach FI. <br /> All work shall be performed in accordance with the attached Proposal dated August 4, 2016 (Exhibit A) <br /> and shall comply with Standards enumerated in the Continuing Contract Agreement for Professional <br /> Services, dated April 18, 2014. <br /> COMPENSATION. <br /> The COUNTY agrees to pay, and Jay Ammon Architect, INC., agrees to accept, $37,175.00 for services <br /> rendered according to the Proposal dated August 2, 2016, identified in the attached EXHIBIT A, <br /> incorporated by reference herein. <br />
