Board of County Commissioners <br /> 1801 27th Street <br /> Vero Beach, Florida 32960 <br /> Telephone: (772) 567-8000 FAX: (772)-778-9391 <br /> Professional Design Services <br /> For <br /> Indian River County Roof Condition Assessments <br /> for the Administrative Annex, Courthouse, and Main Library <br /> WORK ORDER NO. 4 (Facilities Management Division) <br /> FOR <br /> AMENDMENT TO CONTINUING CONTRACT AGREEMENT <br /> FOR PROFESSIONAL SERVICES, APRIL 18, 2014 <br /> WITH <br /> REI ENGINEERS, INC. <br /> All services performed under this contract shall be supervised and certified by a licensed Professional <br /> Engineer(PE) licensed with the State of Florida and staff employed directly with your firm. <br /> Scope of Services <br /> Indian River County is requesting that REI Engineers, Inc., prepare a Roof Condition Assessments for <br /> the Indian River County Administrative Annex, Courthouse, and Main Library. <br /> All work shall be performed in accordance with the attached Proposal dated August 2, 2016 (Exhibit A) <br /> and shall comply with Standards enumerated in the Continuing Contract Agreement for Professional <br /> Services, dated April 18, 2014. <br /> COMPENSATION. <br /> The COUNTY agrees to pay, and REI Engineers, Inc., agrees to accept, $6,575.00 for services rendered <br /> according to the Proposal dated August 2, 2016, identified in the attached EXHIBIT A, incorporated by <br /> reference herein. <br />
