Reinvestment Grant#RFA06H16GS1 <br /> Working Draft <br /> SEFBHN will employ a full time Care Coordinator who will be located at the Connection <br /> Center. The Care Coordinator will work closely with the entire IRCMHC team and our <br /> behavioral health providers to improve transitions from jail to less restrictive community- <br /> based levels of care. They will assist in navigating across systems, link them to needed <br /> services and help remove any barriers to treatment. They will work closely with SEFBHN <br /> Quality Improvement Staff to monitor provider data to ensure appropriate and consistent <br /> diagnoses that support recommended and provided levels of care. The Care Coordinator <br /> will also provide technical assistance and education for our providers. <br /> There will be a Program Manager who will provide leadership, guidance and <br /> administrative supervision to the MHC Case Managers, Licensed Therapist, Care <br /> Manager and the Peer Specialists. The Program Manager's responsibilities will include: <br /> responsibility for ensuring quality services are provided in all assigned programs, and that <br /> program operations are in compliance and running efficiently; providing direct clinical <br /> services to MHC clients, including biopsychosocial evaluations, treatment planning, <br /> individual, group and multi-client counseling activities for clients with co-occurring <br /> disorders; and, maintaining clinical and administrative documentation for assigned clients <br /> as prescribed by agency policy and procedure and by governing law. This position will <br /> require a Master's Degree and a current Florida license in Social Work, Mental Health or <br /> Marriage and Family Counseling, will be employed.by Legacy Behavioral Health and will <br /> be located within Mental Health Court offices <br /> Two Case Managers employed by the IRC Sheriff's office will do both the Pre-screen <br /> and the Forensic Assessments; and, based on their assessments, they will make a <br /> determination of each person's needs and make recommendations to the Court. They will <br /> also conduct an orientation that will include: the MHC program requirements, available <br /> services and resources; and, carefully explain the Court's expectation that the client signs <br /> an agreement to comply with the conditions stipulated in the Order Allowing Defendant to <br /> Be Released ROR with MHC Conditions. In addition, they will make the initial appointment <br /> with the behavioral health care service providers through the Connections Center; they <br /> will problem solve any barriers to services, follow up after the appointment to determine <br /> future steps, and coordinate with the service provider in cases that require more <br /> intervention and planning. <br /> There will be one Care Manager employed and located at the Connections Center. <br /> The Care Manager will conduct a comprehensive needs assessment and make <br /> recommendations to client and/or MHC Team. They will make the initial appointment with <br /> the behavioral health care service providers through the Connections Center and problem <br /> solve any barriers. The Care Manager will follow-up to ensure services are obtained, <br /> determine future steps, and coordinate with the service provider in. cases that require <br /> more intervention and planning. Care Manager will meet with their clients regularly, often <br /> weekly, to monitor their well-being, address their questions or concerns, and connect <br /> them with resources. They will help with transportation issues, housing, supported <br /> employment, education and any other appropriate referrals. They will provide advocacy <br /> and may also recommend support groups for clients or family members to help them cope <br /> with their behavioral health issues. The Care Manager will maintain the appropriate <br /> documentation with detailed notes of their visits with clients, noting their long-term <br /> progress. <br /> i <br /> Page 21 of 33 <br />
