Mehta, P.E., Managing Director, Solid Waste Disposal District'm <br />Subject: Final Pay to Geosyntec for Work Order No. CCNA-2014-Work Order No. 2 <br />Installation of New Groundwater Monitoring Well Clusters Project <br />DESCRIPTIONS AND CONDITIONS: <br />On January 6, 2016, the Florida Department of Environmental Protection issued,a 20 -year renewal <br />permit with a specific condition that the Solid Waste Disposal District (SWDD) install two <br />groundwater monitoring well clusters immediately down -gradient of the existing Cell 1 of the <br />Segment 3 landfill within 90 days of issuance of the permit. <br />On February 2, 2016, the SWDD Board approved Work Order No. 2 to Geosyntec Consultants, Inc. <br />(Geosyntec) in the amount of $19,944 pursuant to an extension and amendment of the Continuing <br />Contract Agreement for Professional Services between Indian River County Solid Waste Disposal <br />District (SWDD) and Geosynetec dated November 4, 2014. The scope of work included the installation <br />of two groundwater monitoring clusters and tasks as outlined in the scope of services. <br />ANALYSIS: <br />Geosyntec has satisfactorily completed all of the tasks described in the scope of work for a total <br />invoiced amount, including the final invoice of $936.79. After final payment, the total amount paid <br />to Geosyntec will be at the cost of $19,932.76 which is under budget. Attached is Geosyntec's <br />project completion report describing in more details the services provided under this work order. <br />FUNDING: <br />Funding for the well installation services is available in the Engineering Services account in the <br />SWDD Landfill Fund account in the SWDD Landfill Fund which is funded from SWDD assessments <br />and user fees.
