ContractNumber, 17-PA-US-10-41-02-081 <br /> Subgrantee/Subrecipient: Indian River County. ^ I - <br /> ' 7i <br /> FIPS/PAID Number 061-99061-00 t nn5 <br /> HURRICANE MATTHEW DISASTER DECLARATION <br /> (FEMA-4283-DR-FL) <br /> Federally Funded Public Assistance State Agreement <br /> THIS AGREEMENT is entered into by the State of Florida, Division of Emergency <br /> Management, whose headquarters is in Tallahassee, Leon County, Florida (hereafter referred to <br /> as the "Grantee/Recipient"), and: <br /> Indian River County <br /> (hereinafter referred to as the "Subgrantee/Subrecipient"). <br /> WHEREAS, <br /> On October 8, 2016, President Barack H Obama issued a Major Disaster Declaration <br /> designated FEMA-4283-DR-FL for the State of Florida as a result of Hurricane Matthew; <br /> and <br /> WHEREAS, <br /> The Declaration, as amended, authorizes Public Assistance funding for eligible disaster <br /> recovery efforts in Bradford, Brevard, Clay, Duval, Flagler, Indian River, Lake, Martin, <br /> Nassau, Palm Beach, Putnam, Seminole, St. Johns, St. Lucie, and Volusia Counties <br /> WHEREAS, <br /> The FEMA-State Agreement executed October 12, 2016, and subsequently amended, <br /> between the State of Florida and the Federal Emergency Management Agency (FEMA) <br /> governing the use of such funds requires the State to share the costs eligible for federal <br /> financial assistance, and the State has undertaken to share those costs, as appropriated, <br /> with its Subgrantees/Subrecipients; and <br /> WHEREAS, <br /> The Subgrantee/Subrecipient is located in the affected area and appears to have suffered <br /> eligible damage, now <br /> THEREFORE, the Grantee/Recipient and the Subgrantee/Subrecipient, as evidenced by the <br /> execution of this document by affixing the signatures of the parties' authorized <br /> representatives below, agree to the following: <br /> 1) DEFINITIONS. <br /> a As used in this Agreement, the following terms have the following meanings unless <br /> another meaning is specified elsewhere: <br /> i Agreement Sum: is the Grantee's/Recipient's amount of reimbursement <br /> to the Subgrantee/Subrecipient for its verified expenses, subject to the <br /> scope of work identified in the Project Worksheet for Small and/or Large <br /> Projects. <br /> ii Eligible activities: are those activities authorized in the FEMA-State <br /> Agreement (located in FloridaPA.org on the main Disaster Summary Page <br /> PA Funding Agreement FEMA-4283-DR-FL page 1 of 23 <br />
