Indian River County <br />Emergency Services District Board of Commissioners <br />4225 43rd Avenue <br />Vero Beach, Florida 32960 <br />Telephone• (772) 226-3900 FAX• 772-567-9323 <br />Project: <br />Fire Station 14, 26th Street and 66th Avenue <br />Preparation and Improvements <br />Emergency Services District <br />WORK ORDER NO. 2 <br />FOR <br />DESIGN/BUILD AGREEMENT <br />PHASE I <br />WITH <br />Barth Construction, Inc <br />This Work Order No 2 is in accordance with the existing DESIGN/BUILD AGREEMENT <br />PHASE I dated December 6, 2012 between Barth Construction, Inc (Design/Builder) and Indian <br />River County (County) This Work Order No 2 amends the agreement as follows <br />SECTION I — PROJECT LIMITS <br />This Work Order No 2 is for the Design/Builder to perform work in connection with the <br />Preparation and Improvements to the proposed Fire Station 14 located at 26th Street and 66th <br />Avenue <br />SECTION II - SCOPE OF SERVICES <br />Completion of site development to include, installation of the fire line and DDCV, connection of <br />the water service to new fire station building, installation of new pad, mount FPL transformer <br />and underground feeders, installation of the remaining site drainage systems and final grading <br />of site, installation and completion of the site concrete drives, parking and sidewalks, completion <br />of the onsite landscape, sod grassing and irrigation system and construction of a 3 bay, <br />prototype fire station building based upon the drawing set dated December 8, 2015 <br />SECTION III — TIME FOR COMPLETION & DELIVERABLES/WORK PRODUCT <br />1. Project shall be completed as follows: <br />a Time for Completion shall be prior to October 1, 2017 <br />SECTION IV — COMPENSATION <br />The County agrees to pay, and the Design/Builder agrees to accept, for the above described <br />services rendered a total amount of $2,446,869 00 <br />Any additional services not described hereon shall be pre -approved by the County <br />1 <br />
