SC -4.02 Subsurface and Physical Conditions <br />SC -4.02 Add the following new paragraphs immediately after paragraph GC -4.02.B: <br />C. In the preparation of Drawings and Specifications, ENGINEER or ENGINEER's <br />Consultants relied upon the following reports of explorations and tests of <br />subsurface conditions at the Site: Roadway Soil Survey Subsoil Soil Exploration <br />and Geotechnical Engineering Evaluation by Ardaman and Associates, Inc. for <br />13 Street SW Bridge at 58th Avenue, May 25, 2007. <br />D. Reports and drawings itemized in SC -4.02.0 are not included with the Bidding <br />Documents. Copies may be examined at Indian River County Administration <br />Building, Public Works, 1801 27th Street, Vero Beach, FL 32960 during <br />regular business hours. These reports and drawings are not part of the Contract <br />Documents. <br />SC -5.01 Performance, Payment and Other Bonds <br />SC -5.01.A. Delete paragraph GC -5.01.A in its entirety and insert the following paragraphs in its <br />place: <br />Within fifteen (15) days of receipt of the Contract Documents for execution, the <br />CONTRACTOR shall furnish a Public Construction Bond in an amount <br />equal to 100% of the Contract Price. <br />1. In lieu of the Public Construction Bond, the CONTRACTOR may furnish <br />an alternative form of security in the form of cash, money order, certified <br />check, cashier's check, irrevocable letter of credit or a security as listed in <br />Part II of F.S. Chapter 625. Any such alternative form of security shall be <br />for the same purpose, and be for the same amount and subject to the <br />same conditions as those applicable to the bond otherwise required. The <br />determination of the value of an alternative form of security shall be made <br />by the OWNER. <br />2. Such Bond shall continue in effect for one (1) year after acceptance of the <br />Work by the OWNER. <br />3. The OWNER shall record, the Public Construction Bond with the Public <br />Record Section of the Indian River County Courthouse located at 2000 <br />16th Avenue, Vero Beach, Florida 32960. <br />SC -5.03 Certificates of Insurance <br />SC -5.03 Delete the second sentence of paragraph GC -5.03 in its entirety. <br />SC -5.04 CONTRACTOR's Liability Insurance <br />SC -5.04 Add the following new paragraphs immediately after paragraph GC -5.04.B: <br />C. The limits of liability for the insurance required by paragraph 5.04 of the General <br />Conditions shall provide coverage for not Tess than the following amounts or <br />greater where required by Laws and Regulations: <br />1. Worker's Compensation: To meet statutory limits in compliance with the <br />Worker's Compensation Law of Florida. This policy must include <br />Employer Liability with a limit $100,000 for each accident, $500,000 <br />disease (policy limit) and $100,000 disease (each employee).
