SPECIAL CONDITIONS FOR PERMIT NO. 14-07 <br />FOR <br />INDIAN RIVER COUNTY PROJECT #0530B <br />(PROPOSED ROADWAY INTERSECTION AND BRIDGE IMPROVEMENTS) <br />AT INTERSECTION OF 13TH STREET, SW AND 58TH AVENUE <br />(LATERAL "B" CANAL) <br />(1) This permit is issuedbased on plans signed and sealed by Michael D. Nixon, P.E., dated <br />April 17, 2014, and bridge plans, prepared by Bridge Design Associates, Inc., dated <br />May 15, 2014. <br />(2) It is noted, and confirmed with Mr. Nixon, that elevation datum shown on above <br />referenced plans refer to 1929 N.G.V.D., and not the North American Vertical Datum, <br />1988, as stated in General Notes (Page 5, of 26) of said plans. <br />To provide adequate access of canal maintenance equipment to Lateral "B" (58th <br />Avenue) and Sub -lateral B-8 (13th Street, SW) Canal, all minimal installation of guard <br />rail shall be as follows: (1) Placed within three (3) feet of top of bank to allow canal <br />maintenance operations over top of guard rail, or; (2) Placed a minimum of eighteen <br />(18) feet away from top of bank to permit operation of maintenance equipment behind <br />proposed guard rail on top of canal bank. <br />(4) During construction phase, no blockage of Lateral "B" (58th Avenue) Canal is <br />permitted. <br />Drainage, in Sub -lateral B-8 (13th Street, SW) Canal shall be maintained/provided <br />throughout construction/installation of R.C.P. culverts. Construction/installation of <br />R.C.P. culverts within Sub -lateral B-8 (13th Street, SW) Canal shall be scheduled during <br />dry season (December through May). If such construction involves temporary blockage <br />of said Sub -lateral B-8 Canal, one or more, pumps having a minimum capacity of <br />24,000 GPM for that portion of project lying west of Lateral "B" (58th Avenue) Canal <br />shall be kept on site and operation to provide immediate by-pass of stormwater <br />discharge around construction site. No by-pass pumping will be required for that <br />portion of Sub -lateral B-8 east of Lateral "B" Canal. <br />(6) All scheduled blockage within said Sub -lateral B-8 Canal shall be coordinated with <br />David E. Gunter, Superintendent of Operation (Phone No. 772-562-2141) on a day -to-. <br />day basis. <br />If a storm event occurs that exceeds the temporary pump by-pass conveyance capacity, <br />or is deemed a necessity for flood control by a representative of Indian River Farms <br />Water Control District or the County, the Contractor shall be prepared, and shall <br />remove any temporary berm, sheeting, etc. blocking said canal flow immediately <br />following such notification. <br />(8) All final fill and grading along Lateral "B" and Sub -lateral B-8 shall be away from top <br />of bank, compacted and stabilized to withstand heavy canal maintenance equipment, <br />and prevent ponding of stormwater and overbank discharge (with resulting erosion) into <br />the District's canals. <br />(3) <br />(5) <br />(7) <br />J:\MEC\SPEC COND=PROVISOS\irfwcd-spec cond Permit # 14-07.doc <br />
