Dale <br />3122 N. 25th Street <br />Fort Pierce, Florida 34946 <br />BOARD OF COUNTY COMMISSIONERS <br />NOTICE OF AWARD <br />Reference: Indian River County Bid No. 2014046 <br />13th Street SW at 58th Avenue Bridge (Project No. 0530B) <br />Dear Mr. Dale: <br />It is my pleasure to inform you that on September 9, 2014, the Board of County Commissioners awarded the <br />above -referenced project to your company. The following documents are required before the applicable County <br />department can issue a "Notice to Proceed" letter. <br />1. Public Construction Bond in the amount of 100% of the contract amount ($1,366,225.20). <br />2. Two Signed Copies of Enclosed Agreement. <br />3. Certificate of Insurance indicating coverage required by Article 5 of the General Conditions (section <br />00700 of the bid documents). Certificate(s) must name Indian River County and Indian River Farms Water <br />Control District as an additional insured and must provide for a 30 day Notice of Cancellation. <br />In accordance with section 255.05(1)(a), Florida Statutes, you are required to execute a Public Construction Bond <br />for the above referenced project. Please submit the Bond, the Certificate(s) of Insurance and two fully -executed <br />copies of the enclosed agreement to this office at the address provided below no later than Wednesday, <br />September 24, 2014.
