BOARD OF COUNTY COMMISSIONERS <br />September 16, 2014 <br />Timothy Rose Contracting, Inc. <br />Attn: Tim Rose <br />1360 SW Old Dixie Highway, Suite 106 <br />Vero Beach, FL 32962 <br />NOTICE OF AWARD <br />Reference: Indian River County Bid No. 2014044 <br />Old Dixie Highway Resurfacing (IRC Project #1137) <br />Dear Mr. Rose: <br />It is my pleasure to inform you that on September 16, 2014, the Board of County Commissioners awarded the <br />above -referenced project to your company. The following documents are required before the applicable County <br />department can issue a "Notice to Proceed" letter. <br />1. Public Construction Bond in the amount of 100% of the contract amount ($1,792,422.35). <br />2. Two Signed Copies of Enclosed Agreement. <br />3. Certificate of Insurance indicating coverage required in Section 5.04 of the Supplemental Conditions — <br />Section 00800 of the bid documents. Certificate(s) must name Indian River County as an additional <br />insured and must provide for a 30 day Notice of Cancellation. <br />In accordance with section 255.05(1)(a), Florida Statutes, you are required to execute a Public Construction Bond <br />for the above referenced project. Please submit the Bond, the Certificate(s) of Insurance and two fully -executed <br />copies of the enclosed agreement to this office at the address provided below no later than Wednesday, October <br />1, 2014.
