Other drilling methods must be <br />approved by the Department prior to well installation. <br />16. A11 wells shall be clearly and permanently labeled and the <br />well site maintained so that the well is visible at all times. <br />Protective barriers must be installed at all wells which may be <br />subject to damage by heavy equipment or traffic. <br />17. <br />An abandonment plan for abandoning any well which is <br />unsuitable for ground water monitoring must be approved by the <br />Department prior to abandonment. <br />REPORTING REQUIREMENTS <br />GENERAL <br />18. Well completion reports for new monitor well(s) must be <br />submitted to the Department on the attached Ground Water <br />Monitoring Well Completion Report Form thirty <br />installation. Note that the latitude and lonitude in degrees, <br />minutes and seconds of each well must be provided on the form. <br />In addition, as -built well construction diagrams and soil boring <br />logs that cover the entire depth of the monitoring wells must be <br />submitted to the Department <br />19. A survey drawing must be submitted within sixty <br />following monitor well installation showing the location (60)days <br />Ball <br />monitor wells (active and abandoned), water bodies and waste <br />filled areas. The location of features on the survey drawing <br />must be horizontally located by standard surveying techniques. <br />The survey drawing shall include the monitor well name and <br />identification number as well as the location and elevation, <br />referenced to NGVD, of all wells, permanent benchmark(s) and/or <br />corner monument marker(s) at the site. The survey shall be <br />conducted and certified by a Florida Registered Surveyor. <br />SEMI-ANNUALLY <br />20. Ground water, surface water and leachate quality analyses <br />shall include the parameters described above. <br />Forms (FDEP Form 62-522.900(2) are attached for Preporting Reportarameter reportinganalyses. In order to facilitate entry of this data into <br />the State computer system, these forms or exact replicas must be <br />used and must not be altered as to content. If these forms are <br />computerized, the completed forms should be submitted on an IBM <br />formatted diskette along with the hardcopy. <br />copies <br />of the forms should be retained so that the n cessaryTheginal information <br />is available to properly complete future reports. The laboratory <br />sheets shall be submitted for all analyses. The semi-annual <br />submittal shall also include Chain of Custody sheets, field data <br />sheets and a summary of any water quality standards or minimum <br />criteria that are exceeded.
