The staff provides screening every <br />two to four months to ensure that babies are developing normally according to established <br />milestones. The program has demonstrated benefits in school readiness, literacy, health and <br />development. <br />Collaboration: Indian River Medical Center, Partners in Women's Health, Kindergarten <br />Readiness Collaborative, Moonshot Moment <br />GOALS/ OUTCOMES REPORT (October 1, 2015- September 30, 2016) <br />Goal #1: Increase parent's knowledge of their child's emerging development and age <br />appropriate child development by 90% <br />Outcome: Overall 95.3% <br />99% of families completed a family centered assessment within 90 days of <br />enrollment <br />92.75% of families have a goal plan within 90 days of enrollment. <br />97.75% of children received a complete development screening within 90 days <br />of enrollment <br />90.25% of children received a complete social -emotional screening within 90 days <br />of enrollment <br />96.75% of children will receive a basic hearing and vision test with general <br />health status update within 90 days of enrollment <br />Goal #2: Improve parents parenting capacity, and parent-child relationships by 80% <br />Outcome: Overall 85.3% <br />86% of parents demonstrate understanding of baby's cues and communication <br />with observed positive responses <br />83.7% of parents develop a reading time with child <br />86.3% of parents demonstrate increased frequency and duration of parent-child <br />interactions <br />P184 <br />
