REQUIRED APPLICATION ATTACHMENTS <br />(In the listed order) <br />1. Application Checklist Form No. 90-22 page (6) <br />2. Completed Assistance Application Form No. 90-22 <br />3. Completed Project Cost Estimate Form No. 90-22 page (5) <br />4. Official Resolution Form No. 90-21 <br />5. Proof of Site Ownership or Control: Provide proof of project elte ownership or <br />control for the required 25 year period. Proof of ownership should he by Attorneys <br />certificate of title. If the project site l.s leased n copy of the lease for the <br />required 25 year period should be attached. All outstanding rights and interests in <br />the area held by others along with known easements deed/lease restrictions and <br />revisionary interests are to identified. <br />6. Location Maps: A map(a) clearly indicating location of the project within the <br />county and/or city. This map should be of sufficient detail so that it can be used <br />to locate the site when driving. <br />7. Site Development Map: A detailed map(a) clearly showing the project site <br />boundaries, existing facilities and proposed project improvements. Project <br />improvements should be clearly shown at a sufficient scale. <br />
