SECTION 00630 - Certificate of Substantial Completion <br />Date of Issuance: , 20 <br />OWNER: Indian River County <br />CONTRACTOR. <br />Project No.:.1139 <br />Project Description: The proposed improvements to CR512 consist of refurbishing (full <br />depth reclamation) of the dual westbound lanes from Roseland Road to US1 and <br />eastbound lanes from Easy Street to US1. Also included with the project is the <br />installation of vehicular detector assemblies. and the signing and marking of the <br />proposed resurfaced lanes. This is a F.D.O.T. Small County Outreach Program (SCOP) <br />funded project, FM No. 4331160-1-58-01. <br />OWNER's Bid No. 2017034 <br />CONTRACT FOR: CR512 WESTBOUND RESURFACING (ROSELAND ROAD TO US1) AND <br />CR512 EASTBOUND RESURFACING (EASY STREET TO US1) <br />This Certificate of Substantial Completion applies to all Work under the Contract Documents or <br />to the following specified parts thereof: <br />To. <br />OWNER <br />And To. <br />CONTRACTOR <br />The Work to which this Certificate applies has been inspected by authorized representatives of <br />OWNER, CONTRACTOR and ENGINEER, and that Work is hereby declared to be <br />substantially complete in accordance with the Contract Documents on <br />DATE OF SUBSTANTIAL COMPLETION <br />A tentative list of items to be completed or corrected is attached hereto. This list may not be all- <br />inclusive, and the failure to include an item in it does not alter the responsibility of <br />CONTRACTOR to complete all the Work in accordance with the Contract Documents.
