BREVARD COUNTY <br />APPLICATION FOR THE TREATMENT AND DISPOSAL OF SEPTAGE <br />Applicant Date <br />Address: <br />Business#: AfterHours# <br />The applicant engages in the servicing of septic tanks and grease traps, and desires to dispose of the <br />septic tank sludge and food service wastes in accordance with current regulations. <br />Brevard County will accept for treatment and disposal septic tank sludge and the waste from food <br />service grease separators, and sewage from Naval ships. <br />REQUIREMENTS of SEPTAGE ACCEPTANCE <br />Applicant must be duly licensed by Brevard County Consumer Health. All vehicles used must <br />meet state and federal regulations governing transport of waste liquid on public roadways. <br />2. Applicant is responsible for any damages to County property or injuries to County personnel <br />arising from the applicant's disposal activities. <br />3. Non-septage discharges wastes are prohibited. other prohibited wastes are: <br />A. Oil, gasoline, diesel fuel, or other flammable liquids, including wastes from automotive <br />service oil separators. <br />B. Portable toilet wastes or wastes from marine holding tanks. <br />C. Wastes from any facility operating under a permit issued by the Florida Department of <br />Environmental Protection. Exceptions will be made on a case by case basis, upon <br />authorization from F.D.E.P. <br />4. Applicant agrees to pay Brevard County Water/Wastewater Division a fee for the treatment and <br />disposal of septage under this application based upon the Board of County Commissioner's <br />approved resolution. <br />Invoices are due upon receipt. Non-payment within forty-five (45) days shall be grounds to <br />terminate the acceptance of septage for treatment. Further non-payment may be grounds for <br />legal action. <br />6. Prior to any disposal activity, the applicant will provide certificate(s) of insurance verifying Auto <br />Liability Insurance with combined limits for Bodily Injury and Property Insurance in an amount <br />no less than $300,000 per occurrence. Insurance must remain in effect and a current certificate <br />be kept on file in Brevard County. <br />7. The applicant shall not assign any interest in this application and shall not transfer any interest <br />without the prior written consent of the County. <br />P223 <br />
