Express Conditions to Issuance of License Agreement <br />45 Days Prior to Event <br />❑ B.1— Submit Application with full disclosure of Events. <br />❑ B.2(a) — Submit 25% Deposit paid upon License Agreement Execution.* <br />❑ B.2(a) — Submit intended alcohol license to be utilized (if applicable). <br />30 Days Prior to Event <br />❑ B.2(a) — Submit Damage Deposit to Fairgrounds Coordinator. <br />❑ B.10 — Submit all certificates of insurance naming "Indian River County, FL" to Fairgrounds <br />Coordinator. Preferably 45 days prior to event, no later than 30 days prior to event. <br />❑ B.10(b) & (j) — Submit any special Participants insurance requirements naming "Indian River <br />County, FL" to Fairgrounds Coordinator and Risk Manager. Preferably 45 days prior to event, no <br />later than 30 days prior to event. <br />❑ B.11 — (If applicable) Submit copies of the alcohol vendor's required state alcoholic beverage <br />license and insurance certificate to the Fairgrounds Coordinator and Risk Manager. <br />Temporary Event Permit only: <br />If you are a bona -fide non-profit civic organization and are obtaining a temporary event permit <br />from the State of Florida Division of Alcoholic Beverages and Tobacco, you will need to provide <br />the temporary event permit. With a temporary event permit, Indian River County Environmental Planning & Code <br />Enforcement Division must sign your temporary event permit. There is a $75 fee associated with <br />this review. Please contact Code Enforcement Officer, Kelly Buck at 772-226-1517 concerning <br />this review. The review process will take approximately 3 days. <br />❑ B.20 — Submit any request for additional staff and Indian River County Sheriff's office deputies <br />needed to Event Coordinator. <br />❑ B.21 — Submit copy of approved "Special Events & Tent Sales Inspection Permit" from the Indian <br />River County Fire Prevention Bureau to Event Coordinator. <br />5 Days Prior to Event <br />❑ B.21 — Submit notification to the Florida Department of Business and Professional Regulation list <br />of all food vendors at least 3 days prior to the event by phone 850-487-1395 or by email at <br />dhr.inlixa my lloridaliccnse.com.
