Bridge over the IRFWCD Sub -Lateral B-8 Canal (58th Avenue) <br />IRC Project No. 0530B <br />76 - <br />DATE: November 21, 2014 <br />DESCRIPTION AND CONDITIONS <br />On December 10, 2013, the Board County Commissioners approved Work Order No. 3 with Bridge Design <br />Associates, Inc. for design of a three -lane bridge on 13th Street S.W. over the Sub -Lateral B-8 Canal (58th <br />Avenue) in the amount of $24,565.00 and construction phase services in the amount of $29,100.00 for a <br />total lump sum of $53,685.00. <br />Bridge Design Associates, Inc. has completed the design for the bridge and has been paid $22,108.50, with <br />$2,210.85 held in retainage. Bridge Design Associates, Inc. has submitted Invoice No. 06-624=R1 dated <br />October 31, 2014 in the amount of $2,456.50 for final design payment of $245.65 and release of retainage of <br />$2,210.85. <br />FUNDING <br />Funds are budgeted and available from Secondary Roads/13th Street, SW/43rd Avenue to 58th Avenue <br />Account No. 10921441-066510-03024 in the amount of $245.65 and from Secondary Roads/13th Street, <br />SW/43rd Avenue to 58th Avenue Retainage Account No. 109-206000-03024 in the amount of $2,210.85. <br />RECOMMENDATION <br />Staff recommends release of retainage to Bridge Design Associates, Inc. for the completed design services <br />and payment of Invoice No. 06-624/R1, dated October 31, 2014 in the amount of $2,456.50. <br />ATTACHMENTS <br />Bridge Design Associates, Inc.
