Florida City Gas <br />FPSC Natural Gas Tariff <br />Volume No 9 Original Sheet No. 13 <br />RULES AND REGULATIONS (Continued) <br />5. GAS LEAKS <br />The Customer shall give immediate notice to the Company of leakage of gas. No <br />deduction on account of leakage shall be required to be made from Customer's bills unless such <br />leakage occurs as the result of fault or neglect of agents of the Company. In case of leakage or <br />fire, the stopcock at the meter should be closed without delay and no light or flame used in the <br />vicinity of the leak. <br />6. CONNECT CHARGE <br />A charge of $80.00 for Residential service or $150.00 for Non -Residential service will be <br />made on the Customer's next bill when gas service is initiated, connected or turned -on. If <br />service is performed, at Customer request, outside of normal business hours the charges shall <br />be $100.00 for Residential service or $200.00 for Non -Residential service. <br />7. RECONNECTION CHARGE <br />A charge of $40.00 for Residential service or $80.00 for Non -Residential service will be <br />made on the. Customer's next bill when gas service is reconnected after disconnection for non- <br />payment of bills. If service is performed, at Customer request, outside of normal business hours <br />the charges shall be $50.00 for Residential service or $100.00 for Non -Residential service. <br />8. FAILED TRIP CHARGE <br />A charge of $20 for Residential and Non -Residential service will . be made on the <br />Customer's next bill when the Customer fails to keep a scheduled appointment with the <br />Company's employee, agent or representative. <br />9. LATE PAYMENT CHARGE <br />A bill shall be considered past due upon the expiration of twenty (20) days from the date <br />of mailing or other delivery thereof by Company. The balance of all past due charges for <br />services rendered are subject to a Late Payment Charge of 1.5% or $5.00 whichever is greater, <br />except that the Late Payment Charge applied to the accounts of federal, state, and local <br />governmental entities, agencies and instrumentalities shall be at a rate no greater than allowed, <br />and in a manner permitted by applicable law. <br />10. RETURNED CHECKS <br />The service charge for each returned check shall be determined in accordance with <br />section 68.065, Florida Statutes. Payment of the full amount of the dishonored payment, plus a <br />service charge of $25 if the face value does not exceed $50, $30 if the face value exceeds $50 <br />but does not exceed $300, $40 if the face value exceeds $300, or 5 percent of the face amount <br />of the dishonored instrument, whichever is greater. <br />Issued by: Carolyn Bermudez Effective: <br />Vice President, Florida City Gas <br />P, 4-32 <br />
