UW -93 -18 -DS <br />BACKGROUND <br />On February 22, 1994, the Indian River County Board of County <br />Commissioners awarded the subject project to Driveways, Inc., in the <br />amount of $34,429.50. (See attached agenda item.) <br />ANALYSIS <br />The project has been completed and accepted per Indian River County <br />Standards and Specifications. It has also been cleared for services by <br />the Department of Environmental Protection. The original contract <br />amount of the project was $34,429.50. The proposed change order (see <br />attached) increases the contract amount by $2,637.92. The increase was <br />due to changes in the location of the water services requested by the <br />homeowners during construction. These changes necessitated additional <br />jack and bore for services, pavement restoration, sodding and other <br />items as shown in the attached Change Order No. 1. <br />By approval of this change order, the contract amount is adjusted to <br />$37,067.42. The contractor is submitting the final pay request <br />including the retainage in the amount of $15,803.12. <br />RECOMMENDATION <br />The Department of Utility Services recommends that the Board of County <br />Commissioners approve the attached Change Order No. 1 in the amount of <br />$2,637.92 and final pay request to Driveways, Inc., in the amount of <br />$15,,803.12.. <br />28 <br />
