Gamble REFERENCES: <br />Executive Director <br />nXNA <br />It is neoamended that the data presented be given formal consideration <br />by the County Commission. <br />The Department of Housing and Urban Development has forwarded four (4) <br />copies of the Annual Contributions Contract (ACC) which has been amended <br />to reflect the replacement of Voucher Project 006 with a new project <br />number 010. <br />The documentation required by HUD for their final approval and execution <br />of the contracts is subni.tted for consideration and approval of the <br />Board of County Canu ssiocne.,rs. <br />(1) Fbur (4) Copies of ACC Part I, Number A3409V of the Annual <br />Contributions Contract for Housing Voucher Rmgrarm. Original <br />signatures are required on all copies of the Contract and no change <br />in its fort is permitted. <br />In addition, per HUD Notice PIH 94-3 dated 1/10/94, renewal, finding inacea is <br />for FY 94 require the submission of two forms that should be attached to <br />the ACC when it is signed and subai.tted to HUD for execution. These forms <br />are: <br />(1) Fbrm HUD -50071, Certification for Contracts, Grants, Loans and <br />Cooperative Agreements. <br />(2) Standard Fbam (SF) -LLL, Disclosure of Lobbying Activities. <br />We respectfully request the County Commission to authorize its Chairman <br />to execute these Contracts for transmittal to the Jacksonville Office of <br />HUD and complete and sign HUD -50071 and (SF) -LLL. <br />ON MOTION by Commissioner Bird, SECONDED by <br />Commissioner Eggert, the Board unanimously <br />authorized the Chairman to execute the <br />documents in connection with the Annual <br />Contributions Contract A3409V, as recommended <br />by staff. <br />PARTIALLY EXECUTED COPIES OF DOCUMENTS <br />ARE ON FILE IN THE OFFICE OF THE CLERK TO THE BOARD <br />13 <br />July 19, 1994 <br />
