�Q� C�:� i riU <br />BOOK <br />RECOMMENDATION <br />Staff recommends the Board approve alternative #3 and requests <br />authorization for the Chairman to sign the certificate for the period <br />of two years as specified in Chapter 304 of the Indian River County <br />Code. <br />The Chairman opened the public hearing and asked if anyone <br />wished to be heard. There being none, he closed the public <br />hearing. <br />ON MOTION by Commissioner Eggert, SECONDED by <br />Commissioner Adams, the Board unanimously <br />approved a Certificate of Public Convenience <br />and Necessity for Able Aviation and Air <br />Ambulance, Inc., to provide ALS/BLS non- <br />emergency interfacility EMS transport service <br />throughout Indian River County for a period of <br />two years, subject to requirements of Chapter <br />304 of the Code; Chapter 401, Florida <br />Statutes; and Chapter 10D-66, Florida <br />Administrative Rule, as set out in staff's <br />Alternate #3. <br />HEALTH INSURANCE - AGENT OF RECORD <br />Administrator Chandler commented from the following: <br />TO: Board of County Commissioners DATE: August 3, 1994 FILE: <br />SUBJECT: Health Insurance - Agent of <br />Record <br />_^ t <br />FROM: James E. Chandler REFERENCES: <br />County Administrator <br />In preparation for implementation of the new health insurance program, <br />effective October 1, 1994, Acordia was notified that the County did not require <br />an agent of -record to service the account. An agent of record, essentially acts <br />on behalf of -the County, where necessary, in servicing the program with the _-- <br />insurer. <br />Since 1983, Schlitt Insurance Services, Inc. has been the agent of record. <br />For the last three years, the fee has been $5,000 per year. Louis Schlitt, <br />President, Schlitt Insurance Services, Inc. does not concur with the staff <br />determination and has requested consideration by the Board. <br />10 <br />August 9, 1994 <br />
