0. Incident Reporting <br />One of the only ways for us to improve in the area of preventing incidents is to learn from the incidents <br />that do happen. The company has gone to great lengths to ensure that we have detailed reporting of <br />incidents. The following are important points to remember: <br />• Report all injuries, accidents, equipment damage, theft, vandalism or property damage, no <br />matter how small <br />• Never go to your own doctorfor injuries suffered during work —this tells us that the injury may <br />not have occurred at work <br />Do not go to a company worker's comp clinic without reporting the injury and receiving <br />direction to go <br />Do not "wait and see" with injuries. If you do not report a work related injury, your claim for it <br />could be denied. <br />Fill out all reports COMPLETELY. Do not skip or omit any information. <br />Seek out the assistance of your Superintendent to help fill out reports. All Superintendents must <br />sign off on all incident reports. <br />The following reports are available from your Foreman, Superintendent or Manager should an incident <br />occur: <br />> JD First Report of Injury & Illness <br />> JD General Liability & Auto Incident Form <br />> JD Utility Damage Report <br />> 1D Lost /Stolen Property Report <br />> JD Vandalism / Damaged Equipment Report <br />> JD Property Damage Report <br />> JD Unaccounted for Damage Report <br />ALL reports must be completely filled out or they will be returned. <br />
