SECTION 00630 - Certificate of Substantial Completion Date of Issuance: 20 OWNER: Indian River County CONTRACTOR CONTRACT FOR: 58TH AVENUE RESURFACING/RECLAMATION FROM NORTH OF 26TH STREET TO SOUTH OF 49TH STREET Project No. IRC -1324 Project Description: The proposed improvements to 581 Avenue from north of 2e Street to south of 491' Street consist of reconstructing the roadway utilizing full depth reclamation, milling and resurfacing, widening the existing shoulders to 5 -feet, structural asphalt, surface asphalt, minor drainage improvements, utility adjustments, signing, and pavement markings. This project is an F.D.O. T. Small County Outreach Program (SCOP) funded project, FM No. 434840-1-54-01. OWNER's Bid No. 2018005 This Certificate of Substantial Completion applies to all Work under the Contract Documents or to the following specified parts thereof: To - OWNER And To - CONTRACTOR The Work to which this Certificate applies has been inspected by authorized representatives of OWNER, CONTRACTOR and ENGINEER, and that Work is hereby declared to be substantially complete in accordance with the Contract Documents on DATE OF SUBSTANTIAL COMPLETION A tentative list of items to be completed or corrected is attached hereto. This list may not be all- inclusive, and the failure to include an item in it does not alter the responsibility of CONTRACTOR to complete all the Work in accordance with the Contract Documents.
