Ennis, P.E., PMP, County Engineer <br />FROM: William Johnson, P.E., Roadway Production Engineer <br />SUBJECT: Work Order No. 3 — Dunkelberger Engineering & Testing, Inc. <br />58th Avenue Resurfacing/Reclamation From 26th Street to 49th Street <br />Testing Services (IRC -1324) <br />DATE: <br />March 05, 2018 <br />DESCRIPTION AND CONDITIONS <br />On September 23, 2014, the Board of County Commissioners approved the annual agreement for <br />Professional Geotechnical Services, with Dunkelberger Engineering and Testing, Inc. for various services <br />including, but not limited to, field and laboratory construction materials testing in Indian River County. This <br />agreement was amended and renewed by the Board of County Commissioners on September 20, 2016. <br />The purpose of Work Order No. 3 is to provide laboratory testing and material engineering related services <br />for the resurfacing/reclamation of 58th Avenue between 26th Street and 491h Street. Dunkelberger <br />Engineering and Testing, Inc. submitted the proposal outlined in the attached Work Order No. 3 and Scope <br />of Services (Exhibit A) for a not -to -exceed amount of $28,956.00. <br />FUNDING <br />Funding is budgeted in the Account Number 10921441-053360-16008 Secondary Roads/FDOT SCOP <br />Grant/58th Avenue Resurfacing (26th St — 57th St) in the amount of $28,956.00. <br />RECOMMENDATION <br />Staff recommends approval of Work Order No. 3 to Dunkelberger Engineering & Testing, Inc. authorizing the <br />professional services as outlined in the Scope of Services (Exhibit A) and requests the Board to authorize the <br />Chairman to execute Work Order No. 3 on their behalf for a not -to -exceed amount of $28,956.00. <br />ATTACHMENTS <br />Work Order No. 3 <br />Scope of Services (Exhibit A) <br />DISTRIBUTION <br />Dunkelberger Engineering & Testing, Inc. <br />APPROVED AGENDA ITEM FOR March 20,201 <br />16 <br />MON <br />
