<br />AN ORDINANCE OF INDIAN RIVER COUNTY,
<br />FLORIDA, ESTABLISHING A DISCLOSURE OF
<br />RELATIONSHIP PROCEDURES FOR ENTITIES DOING
<br />BUSINESS WITH THE COUNTY.
<br />WHEREAS, it is in the public interest to have entities who submit
<br />bids, submit proposals, or enter into contracts with the County to
<br />disclose if there is any relationship with any County Commissioner or
<br />County employee,
<br />NOW, THEREFORE, BE IT ORDAINED BY THE BOARD OF
<br />COUNTY COMMISSIONERS OF INDIAN RIVER COUNTY, that:
<br />SECTION 1. NEW SECTION.
<br />A new Section 105.08 is added to the code to read as follows:
<br />Section 105.08.
<br />Disclosure of relationships.
<br />Any ,entity submitting a bid or proposal or entering into a contract
<br />with the county shall disclose any relationship that may exist between
<br />the contracting entity or an affiliate of the entity and a county
<br />commissioner or county employee.
<br />The relationship with county commissioner or county employee that
<br />must be disclosed is as follows: .
<br />Father, mother, son, daughter, brother, sister,
<br />uncle, aunt, first cousin, nephew, niece, husband,
<br />wife, father-in-law, mother-in-law, daughter-in-law,
<br />son-in-law, brother-in-law, sister-in-law, step-
<br />father, stepmother, stepson, stepdaughter, step-
<br />brother, stepsister, half brother, half sister,
<br />grandparent, or grandchild.
<br />The term "affiliate" includes those officers, directors, executives,
<br />partners, shareholders, employees, members, and agents who are active
<br />in the management of the entity.
<br />The disclosure of relationships shall be a sworn statement made on
<br />a county approved form.
<br />SECTION 2. DISCLOSURE FORM.
<br />The attached disclosure of 'relationships form is hereby adopted as
<br />the official form. of the county for said disclosures. This form shall not
<br />be codified.
