3 Focm 11U_u1u= <br />4/8/88 <br />r , <br />STATE OF FLORIDA DEPARTMENT OF TRANSPORTATION <br />DIVISION OF PRECONSTRUCTION & DESIGN <br />FINAL NEGOTIATION PROCEDURE <br />UTILITY RELOCATION SCHEDULE <br />During the final negotiation stages in clearing a project construction, <br />the District Utilities Section will furnish to the Utility Agency/owner a <br />Utility Relocation Schedule Form 710-010-171, Page 1 of 2, to be completed <br />in sufficient detail to fully describe the Utility Agency/Owner's <br />relocation/adjustment work. The heading will be completed by the District <br />Utilities Section. You will be required to complete, sign, date and <br />return 4 original copies to the District Utilities Section. All <br />information on the Relocation Schedule should be typed and four (four) <br />originals submitted. Page 1 of 2 is self-explanatory. Use as many <br />additional sheets for Blocks "A" and "B" as boxes. Furnish as much <br />information as necessary to describe in detail all work to be performed. <br />Page 2 of 2 will be created by the Utility Agency/Owner which shall show a <br />of the Utility Relocation Schedule It will be the responsibility of the <br />Agency/owner to monitor the advertisement and letting of the highway <br />contract in order to perform work as shown on the Utility Relocatior <br />Schedule to be done before construction begins. <br />This completed Utility Relocation Schedule will be included in the <br />contract bid package. pay close attention to sentence on Pacfe 7 of <br />Listed are some types of contractor activities to which you should relatE <br />to Utility Relocation Schedule. <br />CONSTRUCTION ITEMS (1 ) work days <br />Clearing & Grubbing ( ) work days <br />Grading and/or Drainage (3 ) work days <br />stabilizing ( ) work days <br />Bridge Construction ( ) work days <br />Protecting Existing Utilities ( ) work days <br />Pile driving ( ) work days <br />Traffic Signals ( ) work days <br />Any special notation which is important to your operations can be shows <br />here also. <br />A lump sum number of days for utility adjustment/relocation will be <br />unacceptable without a schedule of activities identifying each operatio,, <br />and its relation to contract time. <br />*NOTE: Attached is sample of Page 2 of 2.
