WORK ORDER NO. 2 101ST AVENUE WIDENING (87th Street to CR 512) IRC -1902 This Work Order Number 2 is entered into as of this 5th day of March , 2019, pursuant to that certain Annual Professional Surveying and Mapping Services Master Agreement dated October 04, 2016 and that certain Renewal of Annual Professional Surveying and Mapping Agreement dated September 18, 2018, collectively referred to as the "Agreement" by and between INDIAN RIVER COUNTY, a political subdivision of the State of Florida ("COUNTY") and CIVILSURV DESIGN GROUP, INC. ("SURVEYOR"). The COUNTY has selected the SURVEYOR to perform the professional services set forth on Exhibit A (Scope of Work), attached to this Work Order No. 2, and made part hereof by this reference. The professional services will be performed by the SURVEYOR for the fee schedule set forth in Exhibit A (CivilSury Design group, Inc. Proposal, dated January 23, 2019), attached to this Work Order and made a part hereof by this reference. The SURVEYOR will perform the professional services within the timeframes more particularly set forth in Exhibit A, attached to this Work Order and made a part hereof by this reference all in accordance with the terms and provisions set forth in the Agreement. Nothing contained in any Work Order shall conflict with the terms of the Agreement and the terms of the Agreement shall be deemed to be incorporated in each individual Work Order as if fully set forth herein.
