Thompson, P.B.pr <br />Capital Projects Manager <br />SUBJECT: Resident Inspection Services for Oslo Road & <br />Old Dixie Highway and CR 510 & SR A1A <br />Intersection Improvements <br />DATE: December 5, 1994 FILE: 510Insp.agn <br />DESCRIPTION AND CONDITIONS <br />Full time resident inspection services on Oslo Road & Old Dixie <br />Highway Intersection Improvements are being performed by a retired <br />FDOT inspector, William Howard. Mr. Howard's services were <br />contracted through ""At Your Service"" at a cost of $22.69/hr., <br />which includes all social security, worker's compensation, <br />unemployment insurance, taxes, and agency fees. The total cost <br />approved by the Board on June 21, 1994 for inspection of Oslo & Old <br />Dixie was $23,597.60. <br />On October 18, 1994, the Board of County Commissioners approved a <br />change order to Dickerson Florida, Inc. that extended the widening <br />on Oslo Road from 500 feet west of Old Dixie Highway to Timber <br />Ridge.
