An acknowledgment that the housing unit meets the applicable local building code and Section 8 Housing <br />Quality Standards, signed and dated by the local building inspector and the local government's housing <br />rehabilitation specialist; <br />c. A copy of the contractor's license; <br />d. A signed statement by the housing unit owner or his or her representative that the work has been completed <br />based on the work write-up and change orders. Should all requirements be fulfilled and the homeowner or <br />his or her representative refuses to acknowledge completion of the work, the housing unit case file shall be <br />documented with a statement detailing the stated reason for said refusal; and <br />e. A 12 -month warranty of work and materials from the contractor to the homeowner that begins when the <br />Certificate of Occupancy or the Certificate of Completion are issued. <br />14. If homes to be rehabilitated with CDBG grant funds will be selected from an existing list of State Housing <br />Initiatives Partnership (SHIP) applicants rather than a public notice soliciting applications, the homes from the <br />SHIP applicants list shall be prioritized using the ranking procedure established in the CDBG HAP. The ranking <br />procedure will be reviewed during monitoring and compared to the list of homes rehabilitated. <br />15. The following data will be provided, by housing unit, as part of the administrative closeout for each activity <br />providing direct benefit (e.g., housing rehabilitation, temporary relocation, hookups, etc.), summarized by activity <br />and submitted with the administrative closeout package: <br />a. Name of each head of household and address of each housing unit rehabilitated with CDBG funds, the date <br />the construction was completed on the housing unit, and the amount of CDBG and non-CDBG funds spent <br />on that housing unit; <br />b. Whether the head of household is female, if the household includes someone who is handicapped or elderly, <br />the number of handicapped persons in the household, the number of elderly persons in the household, and <br />the moderate -income, low-income or very low-income status of the household; <br />c. The number of occupants in the household, categorized by gender; and <br />d. The racial demographics and ethnicity of the head of each household using the following descriptions: <br />1) White, <br />2) African American, <br />3) Asian, <br />4) American Indian or Alaskan Native, <br />5) Native Hawaiian/Pacific Islander, <br />6) American Indian or Alaskan Native and White <br />7) Asian and White, <br />8) African American and White, <br />9) American Indian/Alaskan Native and African American, or <br />10) Other Multi -Racial; and <br />107 <br />45 <br />
