FIRST EXTENSION OF AFFILIATION AGREEMENT BETWEEN <br />THE INDIAN RIVER COUNTY EMERGENCY SERVICES DISTRICT <br />AND THE HEALTH CAREER INSTITUTE, LLC <br />This First Extension of Affiliation Agreement (First Extension) is entered into by and <br />between INDIAN RIVER COUNTY EMERGENCY SERVICES DISTRICT, a dependent special <br />district, whose mailing address is 4225 43rd Avenue, Vero Beach, FL 32967, hereinafter <br />COUNTY and HCI ACQUISITION LLC, a Delaware limited liability doing business as HEALTH <br />CAREER INSTITUTE, LLC, whose location address is 1764 N. Congress Avenue, Suite 203, <br />West Palm Beach, FL 33409, hereinafter HCI do hereby agree as follows: <br />WHEREAS, the COUNTY and HCI entered into an Affiliation Agreement (Agreement) <br />with an effective date of January 16, 2018 for EMT and paramedic students who attend HCI to <br />obtain "on the job training" by conducting ride -a -longs with County Emergency Services District <br />personnel; and <br />WHEREAS, the parties have found the Agreement to be mutually beneficial; and <br />WHEREAS. the Agreement provides for three one year renewals; and <br />WHEREAS, the COUNTY and HCI desire to renew the Agreement for an additional one <br />year period. <br />NOW THEREFORE, the parties do agree as follows: <br />1. The above recitations are true and correct and incorporated herein by reference in this <br />Agreement. <br />2. The Agreement with the effective date of January 16, 2018, is hereby extended until <br />January 15, 2020, nunc pro tunc. <br />3.
