f' <br />SECOND EXTENSION OF AFFILIATION AGREEMENT BETWEEN <br />THE INDIAN RIVER COUNTY EMERGENCY SERVICES DISTRICT <br />AND THE MEDICAL CAREER ACADEMY, INC. <br />This Second Extension of Affiliation Agreement is entered into by and <br />between INDIAN RIVER :COUNTY EMERGENCY SERVICES DISTRICT, a dependent special <br />district, 'whose mailing address is 4225 43rd Avenue, Vero Beach, FL 32.967, hereinafter <br />COUNTY and MEDICAL CAREER ACADEMY, INC., a Florida not for profit corporation, whose <br />new location and mailing address is 7751-1 North Military Trail, Palm Beach Gardens, FL <br />334101300, hereinafter MCA do hereby agree as follows: <br />WHEREAS, the COUNTY and MCA entered into an Affiliation Agreement (Agreement) <br />dated September 20, 2016, for EMT and paramedic students who attend MCA to obtain "on the <br />job training" by conducting ride -a -longs with County Emergency Services District personnel; and <br />WHEREAS, the parties have found the Agreement to be mutually beneficial; and <br />WHEREAS, the Agreement provides for three one year renewals; and <br />WHEREAS, the COUNTY and MCA desire to renew the Agreement for an additional <br />one year period. <br />NOW THEREFORE, the parties do agree as follows: <br />1. The above recitations are true and correct and incorporated herein by reference in this <br />Agreement. <br />2. The Agreement dated September 20, 2016, is hereby extended until September 20, <br />2020. <br />3.
