Lionel Terrace, Stuart, Florida 34997 <br />In reference to Question 7: <br />Has the firm ever been assessed liquidated damages? <br />For a project completed in 2010, our firm was assessed 8 days of liquidated damages for <br />exceeding the allowed contract time. No other exceptions have occurred during our company's <br />thirty two year history. <br />Project: Humiston Park <br />Owner: City of Vero Beach <br />Contact: Matthew Mitts 772-978-4816 <br />In reference to Question8: <br />Has the firm ever been charged by OSHA for violating any OSHA regulations? <br />With a workforce of nearly 100 employees, Sunshine Land Design, Inc. is a labor intensive <br />company working in excess of 5,000 hours a week. With an excellent safety record, our <br />accident to work ratio is exceptional. <br />Each week, our employees conduct tool box talks with handouts pointing specifically to MSDS <br />location and usage; copies of them can be reviewed every Monday at the job site. Our Safety <br />Manager conducts monthly safety meetings with intense emphasis on hazardous conditions. <br />In September 2016, an employee in our landscape division was trimming a palm tree using a <br />chainsaw. Although he had been properly trained on the safe use of a chainsaw and has worked <br />with our company for many years in this position; he was not handling the chainsaw correctly <br />and was cut in the arm. OSHA did issue our company one citation for this incident. We have <br />since abated the citation and ramped up our safety policy to hold individuals accountable for <br />not following our safety policy. <br />Please do not hesitate to contact our office, should you require additional information. <br />Gvww.
