Task 3: Coordination <br />Consultant will prepare permit application Form 62-555.900(1) and submit application <br />with supporting plans and specifications to the Florida Department of Environmental <br />Protection Southeast District. The permit application will include both the bulk sulfuric <br />acid and caustic tank replacement. <br />A permit application fee of $1,000.00, in accordance with FAC 62-4 n.7.a will be <br />furnished by Indian River County Utilities as part of the application. <br />Consultant will answer questions from contractors regarding the scope of work during the <br />price proposal phase. Consultant will review.one set of shop drawings on the tank and <br />piping during the construction phase of the project. <br />TIME SCHEDULE <br />Services for Tasks 1 and 2 are anticipated to be completed within 5 weeks of notice to <br />proceed. Task 3 will be concurrent with project construction and is anticipated to be <br />completed within 6-8 months, depending on contractor's schedule. <br />FEESCHEDULE <br />We will provide these services in accordance with our Continuing Consulting <br />Engineering Services Agreement for Professional Services dated April 17th, 2018, by and <br />between INDIAN RIVER COUNTY, a political subdivision of the State of Florida <br />("COUNTY") and Kimley-Horn and Associates, Inc., ("Consultant"). <br />The Consultant will provide professional services for a lump sum fee as follows: <br />Task No. <br />Task <br />Task Fee <br />Task 1 <br />Review Existing Conditions <br />$ 3,300 <br />Task 2 <br />Design Plans and Specifications <br />$ 13,200 <br />Task 3 <br />Coordination <br />$ 5,500 <br />TOTAL LUMP SUM FEE <br />$ 22,000 <br />ADDITIONAL SERVICES <br />The following services are not included in the Scope of Services for this project, but may <br />be required depending on circumstances that may arise during the execution of this <br />project.
