Work Order No. 26 <br />Hosie Schumann Park <br />Restroom Connections and <br />Utility Service Plans <br />Task 3 - Permitting <br />As part of this scope Kimley Horn will prepare applications and supporting documentation for <br />the necessary permits to construct the restroom facility and utility services. <br />Kimley Horn will submit to the Indian River County Planning Department for administrative <br />approval for the addition of the proposed restroom facility. <br />Kimley Horn will submit for a utility construction permit for potable water and sanitary sewer. <br />This application will consist the equivalent residential use determination, projected water and <br />sewer flows and associated plans. <br />Kimley Horn will prepare the supporting documentation for a St Johns River Water <br />Management District environmental resource permit exemption. Kimley Horn will then submit <br />and coordinate with the District for an exemption determination from the St John River Water <br />Management District for the proposed improvements. <br />Kimley Horn will prepare the supporting documentation to submit for an Indian River County <br />Type A stormwater permit exemption. <br />Due to the limited nature of the work, Kimley Horn does not anticipate more then one round of <br />comments from each of the agencies having jurisdiction above. Kimley-Horn will respond to up <br />to one round of comments from the permitting jurisdictions above as part of this task. <br />In summary the following permits are anticipated: <br />Indian River County Administrative Site Plan Approval <br />Indian River County Utility Construction Permit for Potable Water and Sanitary Sewer <br />St Johns River Water Management District Environmental Resource Permit Exemption <br />Indian County Type A Stormwater Permit exemption <br />Page 3 of 6 <br />
