Work Order No. 27 <br />Pickleball Facilities <br />Dick Bird Park <br />The topographic survey will be performed in accordance with the Standards of Practice set <br />forth in Chapter 5J-17 of the Florida Administrative Code. <br />Kimley-Horn will provide a digital base map of the surveyed property defined above as a base <br />for site construction plan preparation. <br />Task 5 — Permitting Assistance <br />As part of this scope Kimley-Horn will prepare applications and supporting documentation for <br />the following permit applications to construct the pickleball courts: <br />Indian River County Administrative Site Plan Approval <br />St. Johns River Water Management District Environmental Resource Permit <br />Indian County Type A Stormwater Permit Exemption <br />FDEP Exemption for water service to new Water Fountain <br />Kimley-Horn will submit to the Indian River County Planning Department for administrative <br />approval for the addition of the proposed pickleball courts. <br />Kimley-Horn will prepare the supporting documentation for a St. Johns River Water <br />Management District Environmental Resource Permit. Supporting documentation will include <br />the increase of impervious surface, and calculations for accommodating the increase of runoff <br />from the increased impervious area. The calculations will be prepared in a letter format. <br />Kimley-Horn will prepare the supporting documentation to submit for an Indian River County <br />Type A stormwater permit exemption. <br />Due to the nature of the work, Kimley Horn anticipates up to two rounds of comments from <br />each of the agencies having jurisdiction above and will respond to up to two rounds of <br />comments from the permitting jurisdictions above as part of this task. <br />Task 6 — Construction Closeout <br />As part of this scope Kimley-Horn will review the as-builts provided by the COUNTY's selected <br />contractor and provide one round of comments. Once the as-builts are deemed to be in <br />substantial compliance with the approved plans for each of the approvals noted above, Kimley- <br />Horn will prepare final engineering certifications to the agencies listed above. No other <br />construction phase services are included such as responses to requests for information (RFIs), <br />review of shop drawings, site visits. <br />EXHIBIT B — FEE SCHEDULE <br />The COUNTY agrees to pay and the Consultant agrees to accept for services rendered pursuant <br />Page 5 of 7 <br />
