SECOND EXTENSION OF AFFILIATION AGREEMENT BETWEEN <br />THE INDIAN RIVER COUNTY EMERGENCY SERVICES DISTRICT <br />AND THE HEALTH CAREER INSTITUTE, LLC, <br />This Second Extension of Affiliation Agreement (Second Extension) is entered into by and <br />between INDIAN RIVER COUNTY EMERGENCY SERVICES DISTRICT, a dependent special <br />district, whose mailing address is 4225 431d Avenue, Vero Beach, FL 32967, hereinafter COUNTY <br />and HCI ACQUISITION LLC, a Delaware limited liability doing business as HEALTH CAREER <br />INSTITUTE, LLC, whose location address is 1764 N. Congress Ave, Suite 203, West Palm <br />Beach, FL 33409 hereinafter HCI do hereby agree as follows; <br />WHEREAS, the COUNTY and HCI entered into an Affiliation Agreement (Agreement) with <br />an effective date of January 16, 2018, for EMT and paramedic students who attend HCI to obtain <br />"on the job training" by conducting ride -a -longs with County Emergency Services District <br />personnel; and <br />WHEREAS, the parties have found the Agreement to be mutually beneficial; and <br />WHEREAS, the Agreement provides for three one year periods; and <br />WHEREAS, the COUNTY and HCI desire to renew the Agreement for an additional one- <br />year period. <br />NOW THEREFORE, the parties do agree as follows: <br />1. The above recitations are true and correct and incorporated herein by reference in this <br />Agreement. <br />2. The Agreement effective January 16, 2018, is hereby extended until January 15, 2021, <br />nunc pro tunc. <br />3.
