AMENDMENT NUMBER 1 CCNA2018 WORK ORDER NUMBER 7 <br />437717 8TH STREET 24" FM RELOCATION <br />EXHIBIT A <br />The Indian River County Utilities Department has decided to amend Work Order #7 which was <br />approved by the Board of County Commissioners on November 5, 2019. The original project <br />involved Engineering Services in connection with the relocation of an existing 24" force main at 8th <br />Street and 74th Avenue. The Florida Department of Transportation, along with Indian River County, <br />plans to construct a bridge replacement project for the 8th Street Bridge over the Lateral "C" Canal <br />at 74th Avenue in Indian River County, Florida. Said project requires that Indian River County Utilities <br />relocate the existing 24" force main along the south side of 8th Street. <br />The Force Main Relocation project originally contained provisions to prepare and submit permit <br />applications for an Indian River Farms Water Control District Canal Crossing Permit and Indian River <br />County Public Works Right -of -Way Permits for the proposed Road Crossings and Soil Borings. During <br />the course of work to receive a letter of determination from the Florida Department of <br />Environmental Protection FDEP for "No Permit Required" for wastewater collection/transmission <br />permitting, it was determined that the Project will require both an FDEP Wastewater <br />Collection/Transmission System General Permit, an FDEP Notice of Intent to Use and Environmental <br />Resource General Permit, and a U. S. Army Corps of Engineers (USACE) Nationwide Permit for <br />Subaqueous Crossings. Therefore, this Amendment provides for Engineering Services to prepare <br />and submit permit applications for the three (3) above -referenced FDEP and USACE permits. The <br />additional services covered by this Amendment #1 to the project will generally consist of the <br />additional permitting requirements described above. <br />There is no change to the estimated construction cost of the project as described in Work Order 7. <br />A brief description of the scope of services and deliverables for Work Order #7, Amendment #1 is <br />provided below: <br />• Scope of Services <br />Task G —Additional Permitting Services <br />Masteller & Moler, Inc. will provide for permitting services in addition to those defined in Work <br />Order 7 for the following permit applications: <br />• Preparation and submittal of an FDEP Notification/Application for Constructing a Domestic <br />Wastewater Collection/Transmission System -Permit Application; <br />• Preparation and submittal of an FDEP Notice of Intent to Use an Environmental Resource <br />General Permit Application; <br />• Preparation and submittal of a USACE Nationwide Permit for Subaqueous Crossings. <br />77 <br />
