Brown, County Administrator <br />THROUGH: <br />FROM: <br />DATE: <br />Tad Stone, Director <br />Department of Emergency Services <br />Erin Baskins, Staff Assistant IV <br />Department of Emergency Services <br />January 29, 2020 <br />SUBJECT: Request of Name Change from Cleveland Clinic Martin Health for <br />Current Class B Certificate of Public Convenience and Necessity <br />It is respectfully requested that the information contained herein be given formal consideration by <br />the Board of County Commissioners at the next scheduled meeting. <br />Background: <br />On October 1, 2019, the Board approved a Class B Certificate of Public Convenience and <br />Necessity to provide Non -Emergency ambulance inter -facility medical transports at the ALS/BLS <br />level for Cleveland Clinic Martin Health. On January 16, 2020 an application was submitted for a <br />name change from Cleveland Clinic Martin Health to Cleveland Clinic Advanced Medical <br />Transport. All other information and requirements that were provided during the initial application <br />process remained the same. <br />Recommendation: <br />Staff recommends the Board of County Commissioners approve the name change and issue a new <br />Class "B" COPCN certificate to Cleveland Clinic Advanced Medical Transport. <br />Attachments: <br />1.
