Case reporting should be on Adult HIV/AIDS <br />Confidential Case Report CDC Form DH2139 and Pediatric <br />HIV/AIDS Confidential Case Report CDC Form DH2140. <br />Requirements as specified in F.A.C. 64D-2 and 64D-3, F.S. 381 and <br />F.S. 384. Tuberculosis Tuberculosis Program Requirements as specified in F.A.C. 64D-3 <br />and F.S. 392. <br />11. General Communicable Disease Carry out surveillance for reportable communicable and other acute <br />Control diseases, detect outbreaks, respond to individual cases of <br />reportable diseases, investigate outbreaks, and carry out <br />communication and quality assurance functions, as specified in <br />F.A.C. 64D-3, F.S. 381, F.S. 384 and the CHD Epidemiology Guide <br />to Surveillance and Investigations. <br />12.
