TRAINING <br /> To comply,the Senior Resource Association will provide training stated below,as required <br /> ❑ Supervisors who make reasonable suspicion determinations shall receive at least 60 <br /> minutes of training on the physical,behavioral and performance indicators of probable <br /> drug use and 60 minutes on the performance indicators of probable alcohol use. <br /> ❑ Safety-sensitive employees shall be provided a minimum of 120 minutes/2 hours of <br /> orientation and training on this policy and its implementation and on the adverse <br /> effects of drug and alcohol abuse on personal health, safety,and the work <br /> environment. <br /> Any questions regarding this policy or any aspect of this policy or program should contact their <br /> supervisor or Human Resources. <br /> RECORD RETENTION AND RELEASE OF INFORMATION <br /> All dated records and notifications identified by individual will be maintained by the MRO for a <br /> minimum of: <br /> ❑ five(5)years for verified positive controlled substance test results <br /> ❑ one(1)year for negative controlled substance test results <br /> No person may obtain the individual controlled substance test results retained by SRA or the <br /> MRO,and SRA and the MRO shall not release the individual controlled substance test results of <br /> any employee to any person <br /> SRA will maintain records and notifications,as per FTA guidelines: <br /> ❑ by individual for a minimum of five(5)years for verified positive controlled substance <br /> test results <br /> ❑ all dated records for a minimum of one(1) year for verified negative controlled <br /> substance test results and any canceled tests <br /> ❑ by individual for a minimum of five(5)years for verified positive alcohol test results <br /> ❑ all dated records for a minimum of one(1)year for verified negative alcohol test results <br /> ❑ all dated records pertaining to the collection process for two (2)years <br /> ❑ all copies of annual MIS reports submitted to FTA for a minimum of five (5)years <br /> ❑ employee training records for a minimum of two(2)years <br /> ❑ records obtained from previous employer for new hires for a minimum of three (3)years <br /> from the date of the employee's first performance of safety-sensitive duties <br /> No one may obtain the individual alcohol test results retained by SRA or the MRO,and SRA and <br /> the MRO shall not release the individual alcohol test results of any employee to any person <br /> PROGRAM CONTACTS <br /> Any questions regarding this policy or other aspect of the drug-free/alcohol-free <br /> substance abuse program, should contact: <br /> Chief Personnel officer <br /> 4385 43rd avenue <br /> Vero Beach,FL 32967 <br /> Ph.: 772-569-0760 ext. 109 <br /> Fax: 772-778-7272 <br /> 134 <br />
