Flotech <br />Your Valve A Automaton Experts <br />REF; PO# A1730-515 <br />Location; City of Vero Beach <br />1500 P Street SW <br />c/o TLC Diversified <br />Vero Beach, FL 32962 <br />Gary, <br />Our Technician arrived onsite on 9/11/20 and found that the assemblies in question look to have a loose <br />fit in the coupling mounting to the actuator. We found that the limits were not set properly, causing the <br />valve to both open too far and come up short of closing. <br />The Limitorque LY Actuator was leaking oil Into the electrical compartment, causing damage to the <br />electric components of the actuator. <br />The seat Inside of the valve looks to be damaged due to the Incorrect setting of the actuator. <br />We also found that (1) motor seemed to have bearings going bad. This was discovered via the noise the <br />bearings are making. <br />Due to the Issues noted above and the costs that would be incurred to correct the issues found, we are <br />recommending that the best path forward Is to replace the actuators. <br />After evaluation, our technician did a final checkout with the End User and exited the facility. <br />Please let us know if there are any questions or any further Information that is needed. <br />Thank You <br />John Allen <br />Flotech,Inc. <br />
