Director, Parks and Conservation Resources <br />Subject: Amendment No. 2 — Work Order No.1 Donadio & Associates, Architects, P.A. <br />On August 13, 2019, the Board approved Work Order No. 1 for Donadio & Associates, Architects, <br />Inc. (Donadio) to perform work under continuing services contract (RFQ 2018063) for the design and <br />engineering related to elevating the Jones Pier Conservation Area's Main House (1930s Bungalow <br />Cottage). These services are required to complete the design and engineering review by the Florida <br />Division of Emergency Management (FDEM) and subsequent State Historic Preservation Office <br />(SHPO) in order to qualify and receive FEMA mitigation funding to assist with the work necessary to <br />elevate and protect the historic home. <br />Donadio and sub -contractor Schlitt Engineering Services, provided fmal plans on March 16, 2020. A <br />review from the Hazard Mitigation Grant resulted in a necessary change to the design plans which <br />was completed under Amendment Number 1 in the amount of $1300.00. This amendment was <br />approved on May 11, 2020, under the County Administrottor's signing authority. <br />As a result of ongoing site work, the architect has recommended a necessary grading plan be <br />completed by an engineer prior to bidding. This will allow for necessary as-builts and estimating <br />material quantities. Because MBV, Inc. is the engineer of record for Phase 1 improvements to the <br />site, it was determined that they would be able to expedite the grading plan quickly which would also <br />tie in the other site improvements seamlessly. This Amendment Number 2, in the amount of $850.00 <br />will complete the plans necessary for the bid documents. <br />The total cost of the Work Order with both amendments will be $17,900.00. <br />FUNDING: <br />Funding for the amended work order in the amount of $850.00 is available through 004121072- <br />066510-18010 — MTSU Fund/Parks/Jones' Pier Improvements. <br />
